    Way Station's Supported Housing Program provides services and supports to individuals with serious mental illness in community settings, including the individual's home, with the goal of helping them maintain and enhance their level of independence and integration in the community.

    Supported Housing services are provided at a level of intensity and frequency according to each individual's assessed needs and goals. Services include, but are not limited to:

• Assessment of needs and individualized service planning;
• Assistance with daily living needs;
• Medication monitoring;
• Money Management;
• Supportive counseling;
• Personal care skills training;
• Interpersonal and communication skills training;
• Social and leisure skills training and planning;
• Activities that support cultural and spiritual interests;
• Resource management; and
• Crisis intervention.
