Healthy Transition Initiative

(HTI) offers supports to individuals ages 16 to 25 with serious mental illness or emotional disturbance. Way Station’s HTI Team utilizes the principles and practices of the Maryland Model of TAY Supports, Supported Employment (SE) and Assertive Community Treatment (ACT). The following HTI supports are provided to individuals at a frequency, intensity, and duration that address the needs of each individual:

  • Comprehensive assessment of history, needs and interests;
  • Assistance in developing personal goals;
  • Development of an individualized plan;
  • Mobile wrap-around mental health team of mentors, mental health associates and vocational specialists who work closely with each youth;
  • Skill assessment, training and education
  • Support, Linkage and Advocacy.

The goals of Way Station’s HTI are to provide youth and families opportunities to: 

  • Experience a quality of life that includes satisfactory housing, adequate income, positive relationships with friends and family and healthy recreation;
  • Feel empowered, confident and able to advocate for themselves and others as they transition to independent living;
  • Receive strengths based coordinated care for a more seamless transition to adulthood;
  • Be involved in normative activities including employment and/or continuing education and contributing to the community;
  • Have their mental health needs met
  • Feel empowered and supported
  • Have a voice in policy decision making at the state and local levels
  • Provide outreach and support to families and persons within each youths support network
